Care Coordination

Care Coordination is the practice of navigating and integrating the efforts of primary care, specialty healthcare and social service providers to support a patient’s health, wellness, and independence. C4 Therapy requires all new patients to sign a consent form to the primary care doctor for coordination of care. C4 Therapy is required to send the primary care doctor a notification of treatment within 90 days of treatment for new patients. C4 Therapy requires that patient sign primary care doctor consent annually.
